Q. What Insurance do you accept?

Q. How do I make an appointment?

A. Our offices are open from 8:30 a.m. to 5:30 p.m., Monday through Friday. Choice of appointment times will vary depending on physician availability.

While we try to keep waiting time to the minimum, emergencies may occasionally disrupt our schedule. If a significant delay in the doctor’s schedule occurs, the receptionist will try to notify you so that you will have the option of rescheduling your appointment or waiting until the doctor is available. If you are unable to keep a scheduled appointment, please notify us at least 24 hours in advance so that we may accommodate other patients who may need care.

If you are more than 15 minutes late for a scheduled appointment, it may be necessary to reschedule you so as not to inconvenience the other patients. If you are having a medical emergency, please dial 911 immediately.

Q. Do I need a referral?

A. Depending upon the requirements of your insurance plan, a referral from your primary care physician may be necessary to see our physicians. If a referral is required, it is your responsibility to obtain this and it must be presented before you will be able to see the physician. This is a requirement established by your insurance plan that we must follow to be reimbursed for the services our physicians deliver.

We encourage all patients to develop a continuing relationship with a “family physician.” We will gladly coordinate your care with that physician and provide him/her with timely reports concerning your progress.

Q. Where are you located?

A. We have locations in Jefferson Hills, Brentwood, and Bethel Park.

Q. Is there free parking at your facility?

A. Our main office is located on the Jefferson Regional Medical Center Campus and there is a flat rate of $2.00. Valet parking is available for $5.00.

There is no fee for parking at our Bethel Park or Pittsburgh locations.

Q. What forms do I need to fill out for my visit?

A. If this is your first time visiting our office, even if one of our physicians or nurse practitioners has seen you in the hospital, you need to complete a Patient Information form, Notice of Privacy Practice Acknowledgement form, and a Patient History form. These forms are located in the Patient Portal. You will need to request an access code from the office in order to access. If it is more convenient for you to complete at the time of your visit, we suggest you come in 15 minutes prior to your appointment time to complete your information.

Q. What hospitals are you affiliated with?

A. All of our physicians are on staff at Jefferson Regional Medical Center.

Q. Is there anything I need to do before coming to my first appointment?

A. At the time of your first visit, please bring your health insurance information, health insurance co-pay, referrals, list of medications you are currently taking with dosage, list of drug allergies, and other information that might be helpful in diagnosing or treating your problem (such as EKG’s, labs, office notes). If you have not completed the new patient forms available on our website please plan to arrive at least 15 minutes prior to your scheduled appointment to complete these.

Q. How do I refill my medication?

A. For your convenience, ask to have your prescriptions refilled at the time of your office visit. When you have a refill on your prescriptions, call your pharmacy and have them contact us. If you call the office during business hours, you will be asked to leave the refill request with one of our staff members and request will be honored as soon as possible.

Allow 24 hours for prescriptions to be refilled. If prior authorization is required, please allow 72 hours. Mail order prescriptions can either be mailed directly to you, or picked up at our office or faxed if a number is provided.

Q. Insurance/Disability Forms?

A. All Insurance/Disability Forms that are needed will be completed in 5-10 business days. Please provide us with where you would like the form sent once completed. Effective 5/1/14: All disability forms will require a $15.00 fee to be paid prior to completion.
