We participate with many insurance plans and will also bill for non-participating plans for you. Please bring your current insurance cards, so we may copy the front and back of the card for accurate information. It is your responsibility to inform us of any changes in your medical coverage. Please be aware of what your plan covers and your responsibilities for co-pays and deductibles. We also recommend calling your health insurance provider to verify we are participating with your plan.

For your convenience, we accept payment by cash, check, Visa or Mastercard. Patients with co-pays are required to pay them at the time of their visit.

If you are unable to pay at the time of visit, arrangements must be made with the billing department. Patient balances remaining after insurance payments are processed the patient is expected to pay the balance in full within 60 days of the first statement, unless other arrangements are made ahead of time.

All appointments are confirmed. Once confirmation is made, we require prior notice for cancellation of this appointment. If you fail to show for this confirmed appointment a fee of $10 will be assessed before any future appointments will be scheduled. Our office attempts to confirm all appointments. If an emergency arises and you cannot make your appointment, please call our office to cancel so as we may offer that appointment to another patient.


Some of the health care plans require your Primary Care Provider (PCP) to refer you to a specialist for certain services within your network. To avoid having your insurance company deny benefits, please check with your PCP’s office and/or insurance company before obtaining medical care. Without the referral form we may have to reschedule your appointment. Please make sure you have all information necessary for treatment.
